Licensing Program Analysts (LPAs) Dianna Valdez Santana and Vivian Trinh conducted an onsite inspection for the purpose of an Annual Random. Upon arrival LPAs were met by Director, Deepika Wijegunaratne who provided a tour the facility inside and outside. Census was taken in individual classrooms. LPAs observed a total of 5 infant children with 2 staff. During the inspection it was determined the facility is operating within its licensed capacity and within compliance of staffing ratios. A review of the Facility Personnel Report Summary on this date indicates all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. Facility hours are Monday through Friday, 6:30 AM to 6:00 PM. During the inspection of the indoor activity space, LPAs did not observe any items which could pose a danger to children (poisons, detergents, cleaning compounds, medications, or hazardous items). All storage containers for solid waste, including moveable bins, have tight fitting covers that are kept on and are in good repair. Bottles and containers of food brought by the infant\342\200\231s authorized representatives are labeled with the infant\342\200\231s name and date. Infant changing tables are placed within arm\342\200\231s reach of a sink. The facility has age-appropriate furniture and equipment including, but no limited to, cribs, or mats, changing tables and feeding chairs. The facility has sufficient infant napping equipment that meets the requirements. A baby walker was not observed during today\342\200\231s visit. Facility does not provide food for the infants; it is provided by parents. Food is properly stored. Floors, equipment, and furniture were clean and were observed to be in good repair and free of sharp edges. There is drinking water available to children indoors by water dispenser and the children have their individual drinking bottles with their name labeled on it. Page 1 of 5 Page 2 The children's changing tables were clean and sanitary. Children nap in cribs or on mats, and bedding is stored individually and is washed daily by parents. The facility has conducted an emergency drill within the past six months. A Fire Drill was last conducted on 11/17/23. The facility has working carbon monoxide detectors and fire extinguishers and fire alarm. Facility met all posting requirements. The California Child Passenger Safety Law was posted by the entrance of the facility. Staff files were reviewed for staff present during the facility inspection on this date; 5 staff files were reviewed. Health screening and immunizations as required were reviewed. Beginning September 1, 2016, Health and Safety (H&S) 1597.622 states, a person shall not be employed or volunteer at a childcare center if he or she has not been immunized against influenza, pertussis, and measles. Proof of immunization against pertussis, measles for 5 staff were reviewed and 5 out of 5 were within compliance. Beginning March 31, 2018, H&S Code 1596.8662 requires all directors and employees to complete mandated reporting training, and to renew the training every two years. 2 out of 5 staff did not have a current Mandated Reporter Training Certificates. Staff present inside infant program possesses current EMSA approved Pediatric CPR/First Aid certifications, which expires 03/2025. Children's records were reviewed, and there was a separate, complete, and current record for each child. A random sample of 5 children\342\200\231s files were reviewed for documentation of the child\342\200\231s name, address, and telephone number of the child\342\200\231s authorized representative and of relatives or others that can assume responsibility for the child if the authorized representative cannot be reached when necessary (LIC 700) and a medical assessment. LPAs reviewed the napping log for infants up to 24 months and LIC 9227 Infant Safe Sleep Plan for infants up to 12 months. In the areas reviewed, the children\342\200\231s files, 2 out of 5 were found to not be in compliance. Sign in/out procedure was reviewed for compliance. LPA also informed director of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. Director was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated. Based on LPAs\342\200\231 observations, and record reviews the following violations were observed and two type Bs are being cited in accordance with California Code of Regulations, Title 22, Division 12, Chapter 3, Health and Safety Code 1596.8662(b)(1) and CCR 101220(a) is being cited on the attached LIC 809D.
