What the inspector wrote
On 7/29/2025 Licensing Program Analyst (LPA) Dianna Valdez Santana conducted an onsite inspection for the purpose of a Random Annual Inspection. Upon arrival LPA was met by Director, Deepika Wijegunaratne who provided a tour the facility inside and outside. Census was taken in individual classrooms, Room #4 had 3 infant children with 1 staff. Later at approximately 9:15am one additional staff and one infant arrived in Room #7. The total census was 4 infants and 2 staff. During the inspection it was determined the facility is operating within its licensed capacity and within compliance of staffing ratios. A review of the Facility Personnel Report Summary on this date indicates all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. Facility hours are Monday through Friday, 6:30 AM to 6:00 PM.
During the inspection of the indoor activity space, LPA did not observe any items which could pose a danger to children (poisons, detergents, cleaning compounds, medications, or hazardous items). All storage containers for solid waste, including moveable bins, have tight fitting covers that are kept on and are in good repair. Bottles and containers of food are brought by the infant\342\200\231s authorized representatives and are labeled with the infant\342\200\231s name and date. Infant changing tables are placed within arm\342\200\231s reach of a sink. The facility has age-appropriate furniture and equipment including, but not limited to, cribs, or mats, changing table and feeding chairs. The facility has sufficient infant napping equipment that meets the requirements. A baby walker was not observed during today\342\200\231s visit.
Facility does not provide food for the infants; it is provided by parents. Food is properly stored. Floors, equipment, and furniture were clean and were observed to be in good repair and free of sharp edges.

There is drinking water available to children indoors by water dispenser and the children have their individual drinking bottles with their name labeled on them.
The children's changing tables were clean and sanitary. Children nap in cribs or on mats, and bedding is stored individually and is washed weekly by parents. The facility has conducted an emergency drill within the past six months. A Fire Drill was last conducted on 6/23/25. The facility has a working carbon monoxide detector and fire extinguishers and fire alarm. Facility met all posting requirements. The California Child Passenger Safety Law was posted by the entrance of the facility.
Staff files were reviewed for staff present during the facility inspection this date; 3 staff files were reviewed. Health screening and immunizations as required were reviewed. Beginning September 1, 2016, Health and Safety (H&S) 1597.622 states, a person shall not be employed or volunteer at a childcare center if he or she has not been immunized against influenza, pertussis, and measles. Proof of immunization against pertussis and measles for 3 staff were reviewed and 3 (three) out of 3 (three) were within compliance.
Beginning March 31, 2018, H&S Code 1596.8662 requires all directors and employees to complete mandated reporting training, and to renew the training every two years. Three (3) out of three (3) staff had current Mandated Reporter Training Certificates. Director\342\200\231s Mandated Reporter Training Certificate expires 06/16/2026. Staff present inside infant program possesses current American Red Cross Pediatric CPR/First Aid certifications, which expires 9/2026.
Children's records were reviewed, and there was a separate, complete, and current record for each child. A random sample of 5 children\342\200\231s files were reviewed for documentation of the child\342\200\231s name, address, and telephone number of the child\342\200\231s authorized representative and of relatives or others that can assume responsibility for the child if the authorized representative cannot be reached when necessary (LIC 700) and a medical assessment. LPA reviewed the napping log for infants up to 24 months and the LIC 9227 Infant Safe Sleep Plan for infants up to 12 months. In the areas reviewed, the children\342\200\231s files were found to be in full compliance. Sign in/out procedure was reviewed and found within compliance.

A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

In the areas that were evaluated, no deficiencies were observed of the California Code of Regulations, Title 22, Division 12 at the time of the visit.

Exit interview was conducted and report was reviewed with the Director Deepika Wijegunaratne. A notice of site visit was given and must remain posted for 30 days. Appeal Rights were provided and discussed. Director was provided a copy of their appeal rights (LIC 9058) and their signature on this form acknowledges receipt of these rights.
All appeals must be in writing and received by the Regional Office within 15 business days.
