What the inspector wrote

Page 2 During the course of investigation, LPA interviewed Director and 5 staff, 3 parents, and reviewed records. During the initial inspection dated 11/18/2022, LPA\342\200\231s observed staff supervising children, each room was in ratio, children were engaged in activities. 5 out of 5 staff stated they are not left alone with more than 4 infants. 5 out of 5 staff stated they are never out of ratio. 5 out of 5 staff are aware of the ratios needed for compliance for infant, preschool and toddler classrooms. Director stated the following: to ensure s taff the facility is meeting the ratio requirement, staff will call the office to inform us that they are in ratio. If there\342\200\231s many children in the classroom, we will send some children to a different classroom or we will add teachers to their room. But usually we have more than enough staff for coverage. LPA Duron contacted five parents by phone and was able to interview 3 parents. All interviewed parents stated they did not have any concern with facility. On 2/10/2023, LPA Duron arrived at the facility at 8am and observed 6 children and 1 staff. Facility was in compliance with the ratio requirement. During the investigation, LPA Duron randomly reviewed staff timesheets and children sign in and out sheets dated 11/14/22. On 11/14/22, between 7-8am sign in/ sign out sheets reviewed 3 children with 1 teacher. Facility was within the ratio requirement during the reviewed date. Based on the information gathered from LPAs\342\200\231 interviews, observation, and reviewing records, there is insufficient evidence to corroborate the allegations of staff were out of ratio; facility does not have adequate staffing. Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the allegations did or did not occur in the day care facility, therefore the allegations are UNSUBSTANTIATED. An exit interview and report reviewed with Director Natalie Hernandez. Director was provided a copy of their appeal rights and their signature on this form acknowledges receipt of these rights. All appeals must be in writing and received by the Licensing office within 15 business days. The facility representative was informed that the \342\200\234Notice of Site Visit\342\200\235 must be posted for 30 consecutive days. Failure to post will result in Civil Penalties of $100.00. The \342\200\234Notice of Site Visit\342\200\235 must be posted on or adjacent to the door.
