On 3/10/2025 an Unusual Incident Report was filed with the Department to self-report an incident that occurred on 3/6/2025, Child #1 C1, who was found in the parking lot outside the gate, left unattended. During the course of the investigation, LPA interviewed Center Director Marge Menashe, and 4 staff members, attempted to interview the children, obtained a copy of the children's roster, review Child #1 file, sign-in/out on the day of the incident. Due to additional Interview information needed, the report incident needs further investigation. An exit interview was conducted. The Notice of Site Visit was posted for at least 30 consecutive days. Appeal Rights were explained. The End of Report.
