What the inspector wrote

On 4/22/25, at 12:30pm Licensing Program Analyst (LPA) Anna Chan conducted a Plan of Correction (POC) inspection in response to a Type B violation issued on 04/3/25 101217(d) All personnel records shall be maintained at the child care center and shall be available to the licensing agency for review. Upon arrival, LPA met with Director Kelsie Nanez. Census was taken. Overall census was 58 Preschool children and 6 staff. The deficiency that was previously cited on a LIC 809D dated 04/03/25 has been cleared. However, during record review, LPA discovered that 4 of 5 staff files are missing TB Tests and 3 of 5 staff files are missing LIC503 Health Screening. In the areas that were evaluated and based on records reviewed, the facility was not in compliance with the California Code of Regulations, Title 22, Division 12 Section 101216(g)(1). See LIC809D for deficiency cited. Exit interview conducted and report was reviewed with Director Kelsie Nanez. Appeal rights was provided and deficiency was discussed. Notice of site visit posted. Director was reminded that Notice of Site visit must be posted for 30 consecutive days.
