What the inspector wrote

On 04/22/25, at 1:35pm Licensing Program Analyst (LPA), Anna Chan conducted an onsite inspection for the purpose of an unannounced Annual Random Visit. LPA met with Director Kelsie Nanez and toured the facility inside and outside and the floor and yard plan were verified. The facility transports children to and from school. At 2:17pm 9 school-age children arrived and at 2:25pm another 7 children arrived at the facility and there were 2 staff in the classroom. The facility is located across 24hour Fitness gym in Anaheim Hills Festival. It was determined the facility is operating within its licensed capacity and within compliance of staffing ratios. Facility hours are 6:30a.m.- 6:00p.m., Monday through Fridays. A review of the Facility Personnel Report Summary on this date indicates all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. During the inspection of the indoor activity space, items which could pose a danger to children (detergents, cleaning compounds, and medications) were observed to be stored out of the reach of children. Poisons/Hazardous Items are not kept on the premises. Food is prepared on site; PM snacks are provided. Wednesday lunches are provided when school has short days. Food prep areas were clean and sanitary. Food is properly stored.; Menus were posted near the entrance walkway to the office and at the kitchen area where it could be seen by parents. Allergy list of children are also posted in the kitchen and in the classrooms. Page 1 of 3 Floors, equipment, and furniture were clean and were observed to be in good repair and free of sharp edges. There is a purified drinking water available to children indoors with disposable cups and children have their own sports bottles properly labeled with their names on it. There are 2 bathrooms in inside the classroom. Bathrooms were observed to be clean and sanitary. The facility has a working carbon monoxide detector and fire extinguisher The outdoor activity space was inspected for compliance. The cushioning is a black top cushioning around the climbing equipment which appeared to be enough to absorbs falls. Drinking water in the outdoor activity space is provided by purified water gallon and disposable cups. The outdoor equipment and toys were in good repair and free of sharp edges. There are no bodies of water present at the facility. The facility ground was safe and sanitary. The facility has a working carbon monoxide detector and fire extinguisher. The facility has conducted an emergency drill within the past six months on April 15, 2025. Facility met all posting requirement. The California Child Passenger Safety Law was posted by the entrance of the facility 2 Staff files were reviewed for staff present during the facility inspection on this date. Health screening and immunization as required were reviewed. Beginning September 1, 2016, Health and Safety (H&S) 1596.7995 (a)(1) a person shall not be employed or volunteer at a day care center if he or she has not been immunized against influenza, pertussis, and measles. Each employee and volunteer shall receive an influenza vaccination between August 1 and December 1 of each year. Proof of immunization against pertussis, measles for the facility were reviewed and within compliance. Beginning March 31, 2018, H&S Code 1596.8662 requires all directors and employees to complete mandated reporter training, and to renew the training every two years. At least one staff member present possesses current EMSA approved Pediatric CPR/First Aid certifications, which expires 10/2025. Page 2 of 3 5 Children's records were reviewed, and there was a separate, complete, and current record for each child. In the areas Sign in/out was reviewed for compliance. Each children representative has their own login ID. Children are being dropped off at the facility between 6:30am and 7:30am. This facility provides Incidental Medical Services (IMS) LPA reviewed storage of medication and equipment/supplies, and reviewed children\342\200\231s, personnel, and administrative records. An Inspection and exit interview were completed with Director, Kelsie Nanez. The report was reviewed and discussed. Notice of site visit was posted.
