What the inspector wrote

An unannounced case management inspection was conducted on 12/01/2025 by Licensing Program Analyst (LPA), Deschampe. LPA arrived at the facility at 9:04 AM in response to a self-reported Unusual Incident Report (UIR) dated 11/10/2025. Present during today\342\200\231s inspection was the Facility Representative/Director (FR), Leisha Dearing. FR guided LPA on a tour of the facility. Census was taken as follows: 47 children with 7 staff. A review of staff criminal records indicates all facility staff or individuals who require caregiver background checks have received a criminal record clearance or exemption and a child abuse index clearance. On 11/10/2025, the UIR was filed with the Orange County Regional Office (OCRO) to self-report an incident with an unknown date and time. Director stated on the report that Child 1 (C1) alleged Child 2 (C2) inappropriately touched C1 in the bathroom. LPA received a copy of the UIR on 11/10/2025. Per the UIR, director reported C1 alleged C2 inappropriately touched C1 in the bathroom. C1 allegedly told staff. Director interviewed staff. Staff stated they have never seen an unusual incident in the bathroom. Staff stated C1 did not disclose any information to staff regarding an unusual incident. On this date, LPA made observations, received facility records, interviewed children, and conducted interviews with 5 staff. Due to additional information needed, the above allegation needs further investigation. Appeal Rights were discussed. The facility representative was provided a copy of their appeal rights (LIC 9058) and their signature on this form acknowledges receipt of these rights. All appeals must be in writing and received by the OCRO within 15 business days. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the Facility Representative, Leisha Dearing. End of report.
