What the inspector wrote

In continuation of the case management inspection initiated on 12/01/2025 in response to a self-reported Unusual Incident Report (UIR) dated 11/10/2025, Licensing Program Analyst (LPA), Deschampe conducted an unannounced case management inspection on this date, 02/05/2026 with arrival at the facility at 9:18 AM. Present during today\342\200\231s inspection was the Facility Representative/Director (FR), Leisha Dearing. Total enrollment is 34, with 20 children and 3 staff at the time census was taken, due to varied schedules. A review of staff criminal records on this date 02/05/2026 indicates all facility staff or individuals who require caregiver background checks have received a criminal record clearance or exemption and a child abuse index clearance. On 11/10/2025, the UIR was filed with the Orange County Regional Office (OCRO) to self-report an incident with an unknown date and time. Director stated on the report that Child 1 (C1) alleged Child 2 (C2) inappropriately touched C1 in the bathroom. Director reported Parent 1 (P1) stated P1 overreacted and no police report was filed. LPA made observations, reviewed facility records, and conducted interviews with 5 staff. No children interviews were conducted due to not qualifying for the Ten Step Investigative Interview questions. During staff interviews, 5 out of 5 staff stated they have never witnessed inappropriate behavior between C1 and C2. 5 out of 5 staff stated visual supervision is always maintained so there was not an opportunity for the incident to have happened. 5 out of 5 staff do not think the incident happened. No deficiencies cited during this visit. The facility representative was provided a copy of their appeal rights (LIC9058) and their signature on this form acknowledges receipt of these rights. All appeals must be in writing and received by the Regional Office within 15 business days. The "Notice of Site Visit" was posted and must remain posted for 30 consecutive days. Exit interview conducted and report was reviewed with the Facility Representative, Leisha Dearing.
