What the inspector wrote

An inspection was conducted at the facility by Licensing Program Analyst (LPA) Nguyen Nick Tran. LPA met with Director Esther Lee, who guided LPA Tran on a tour of the facility. Census was taken. Observed was a total of 49 preschool children supervised by 5 staff members. The facility floor and yard plan were verified. During today's inspection it was determined the facility is operating within its licensed capacity and within compliance of staffing ratios. LPA informed the Director the purpose of the visit. A review of the Facility Personnel Report Summary on this date 05/22/2024 indicates all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. Director was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated. The facility has requested to decrease the capacity of the preschool capacity by 18 and add 18 toddler options (18 months to 36 months), in Room #1. An approved fire clearance was received by Licensing on 04/08/24, approving room #1 for 18 toddler options to be added. During today's inspection, LPA observed Room #1 has one sink inside the classroom and a changing table within an arm reach of the hand washing sink. In addition, facility has a restroom in the building for the toddlers to use as needed. This restroom has been designated for toddler or preschool child use only. Room #1 is measured as followed: 425 sq. ft./35 = 12 children. The toddlers can walk through a hallway to the outdoor area and use playground that is designated for toddler use, with sufficient space, and has age appropriate play equipments Toddler playground is measured as followed: 900 sq. ft./75 = 12 children. (Continue next page) (Page 2 of Report) Facility's Director will submit updated documentation for the addition of the 12 toddler options. Operating hours of the toddler option and preschool are 7am to 6pm, Monday - Friday. Indoor activities space was inspected. The items which could pose a danger to children (detergents, cleaning compounds, and medications) were stored out of the reach of children. Food is prepared on site; snacks are provided.Food prep areas appear clean and sanitary, food is properly stored. The facility has a menu posted and is up to date. Facility has conducted lead testing for their water outlets and have submitted the result. There is drinking water available to children both indoors and outdoors. The children's bathrooms are clean and sanitary. There is a changing table available in the toddler room. Children nap on mats, bedding is stored individually and is taken home weekly to be washed by parents. The facility has conducted an emergency drill within the past six months, and is documenting them. The facility has a working smoke detector, carbon monoxide detector, and fire extinguisher. The playgrounds are completely fenced and fencing is in good repair. Proof of immunization's against pertussis, influenza (or written declination), and measles for all employees/volunteers are on file and within compliance. Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. LPA also informed Director of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. The facility is approved for 12 toddlers options to be added in room #1, pending facility's updated application and manager's review. There were no Title 22 deficiencies cited during today's inspection. Appeal Rights were discussed. The facility representative was provided a copy of their appeal rights (LIC 9058) and their signature on this form acknowledges receipt of these rights. All appeals must be in writing and received by the Regional Office within 15 business days. A notice of site visit was given and must remain posted for 30 days.
