What Happened
3-year inspection found facility operating within licensed capacity with generally compliant conditions. Four of five staff lack required immunizations against influenza, pertussis, and measles, and two staff have expired mandated reporter training certificates.

Based on observation, interview, and record review, the facility did not comply with the section cited above. During staff file review, LPA observed the S2 & S5 missing the current mandated reporter certification. Director was unable to provide the current mandated reporter certification which poses/posed a potential health, safety or personal rights risk to persons in care.
