Licensing Program Analyst (LPA) Carreiro conducted an unannounced Case Management visit to deliver the Amended Report for a report that was previously completed at the facility on 10/23/2025. At 9:00am LPA met with Substitute Director Oana Elias, who guided LPA on tour of the facility. The overall census observed was 15 preschool children and 4 preschool staff members. A review of the Facility Personnel Report Summary on 12/19/2025 indicates all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. During visit, LPA Carreiro discussed Amended report with the Substitute Director Oana Elias. Substitute Director Oana Elias acknowledged and signed Amended report. A copy of the Amended report was provided to the Substitute Director Oana Elias. Appeal Rights were explained. The Substitute Director Oana Elias was provided with a copy of appeal rights (LIC 9058) and their signature on this form acknowledges receipt of these rights. All appeals must be in writing and received by the Regional Office within 15 business days. First level appeals should be sent to the regional manager to the address listed above. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with Substitute Director Oana Elias.
