What the inspector wrote

On 9/03/2025 at 8:05am, Licensing Program Analyst (LPA) Sarah Garcia conducted a case management-licensee initiated visit to inspect the facility for the purpose of adjusting age range. At 8:15am, LPA observed 16 school age children with 2 staff including director leaving the facility to walk children to school approximately 800 feet away. During the inspection it was determined the facility is operating within its licensed capacity and within compliance of staffing ratios. At 8:30am, LPA met with director, Cynthia Okert. Director guided LPA on a walk through of the facility. A review of the Facility Personnel Report Summary on this date indicates all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. On 7/25/2025, Director informed LPA that the facility will be initiating an age change to the school age program. The facility plans to serve 65 school age children from transitional kindergarten to 12th grade in Rooms 201, 202, 203, 204, 205, youth center, sanctuary, and social hall. The capacity will remain 65 children. LPA received and reviewed the facility sketch (LIC 999A) and the fire inspection (STD 850) was approved by the City of Fullerton Fire Inspection Services on 8/22/2025. Continued on Page 2 Page 2 LPA inspected Rooms 201, 202, 203, 204, 205, youth center, sanctuary, and social hall and observed all indoor activity spaces to be complete with safe, age-appropriate furniture and equipment, including tables, chairs, cubbies, bookshelves, and other activity supplies for children. Filtered drinking water is available in the classrooms. LPA did not observe any hazardous items accessible to children. LPA inspected the children's bathrooms and observed them to be safe, sanitary, and in good repair. LPA inspected the kitchen area and observed the space to be clean and orderly. LPA observed the outdoor activity space which includes the playground and patio area. The outdoor equipment and toys were in good repair and free of sharp edges. The outdoor areas were safe, sanitary, and in good repair. There are no bodies of water or weapons on the property. File will be submitted to management for final approval. Exit interview conducted and report was reviewed with the director, Cynthia Okert. No deficiencies cited. A notice of site visit was given and must remain posted for 30 days. End of Report
