What the inspector wrote

On 07/07/2025, a case management inspection was conducted today by Licensing Program Analysts (LPAs), K. Navar and K. Trinh who met with the Site Supervisor, Melanie, Myers. A self-reported incident by the facility was received at the regional office on 5/28/25. Upon arrival there were 5 (five) staff present and 38 school age children in care. A review of criminal record clearances indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. On 5/28/2025 the regional office received a UIR stating that C1 had arrived at school not feeling well and was offered a snack that had traces of egg in it. During this investigation, LPA conducted a facility inspection, interviewed #1 (one) staff (S1), and obtained a copy of C1’s Pre-Admission Health History. During records review of C1’s preadmission health history LPAs reviewed that C1 had an allergy to eggs. #1 (one) staff interviewed stated that C1 was given a Hawaiian roll that had an ingredient of egg in it. S1 stated that snack menus for the month are posted and available for parents to review and that they were never told that C1 had an allergy of traces of egg and never was told C1 can not have a Hawaiian roll. S1 stated they have now included detailed information for children enrolled with allergies and that staff #2 (two) will audit preadmission health history forms. During facility inspection LPA observed a current snack menu posted in 4 (four) classrooms Room 1, Room 2, Room 3, and Room 4. LPA’s observed documentation and storage of medication to be individually stored for 8 (eight) children enrolled, a current allergy list/ life threatening allergy list posted on the fridge with children names and allergy information. The facility was in compliance of the California Code of Regulations, Title 22, Division 12 and no citations will be issued today. Exit interview conducted and report was reviewed with the Site Supervisor, Melanie, Myers. A notice of site visit was given and must remain posted for 30 days.
