What the inspector wrote

On 2/13/2026, Licensing Program Analyst (LPA), Carreiro conducted an unannounced subsequent visit to the facility to follow up on an incident that was self-reported to the Department. Upon arrival, LPA met with the Director MYERS, MELANIE. LPA disclosed the purpose of the inspection and was led on a tour of the facility. There were 20 children and 7 staff members present. Hours of operation are Monday- Friday 7:00am-6:00pm. A review of the Facility Personnel Report Summary on this date 2/13/2026 indicates all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. On 12/19/2025, Regional Office received a self-reported Unusual Incident Report (UIR) indicating on 12/12/25, Child #1’(C1) representative came to the center and spoke with Director. C1’s representative stated there was a negative interaction between C1 and Staff #1(S1). It was reported that during a group activity, C1 stated that S1 was yelling at C1 when C1 asked for help. Director also reported that S1 is placed on leave because of the situation. During the course of investigation, LPA interviewed five (5) staff members and three (3) children. Three (3) out of five (5) interviewed staff stated they witnessed and/or have heard S1 yelling at children in care. Two (2) out of three (3) children stated that S1 yelled at them. The Director contacted their supervisor and HR after speaking with the C1’s parent. Continue to page 2 Page 2 The facility informed the LPA that S1’s employment was terminated as of 1/13/2026 as a result of an internal investigation. Based on the interviews conducted and the self-reported incident, violation of personal rights was observed and being cited on the LIC809D. Exit interview conducted and report was reviewed with the Director MYERS, MELANIE. A notice of site visit was given and must remain posted for 30 days.
