What the inspector wrote

During the investigation LPA Chan interviewed staff. On 8/16/23 LPA interviewed 5 staff and all 5 staff have similar responses when asked about beddings being kept in a plastic bag and does not come into contact with other children’s belongings. Staff 1 (S1) stated that after nap, the teachers help the children put the sheets and blankets on a separate plastic bag. Staff 2 (S2) stated a teacher will transition the child to the other classroom while the other teacher cleans up the bedding and puts them away in a separate plastic. Staff 3 (S3) stated the children start taking off the sheets, put in their bags and put back in the cubbies and put back the beds/cots where they store them. Staff 4 (S4) stated that older kids make their own beds, and they assist them putting it back in the bag and assist carrying the beds. Staff 5 (S5) stated when the children wake up one at a time, they assist the children to take away with the sheets and blankets and put it back in the plastic bags. On 8/16/23 LPA observed beddings such as sheets and blankets for children were placed in a separate Ziploc bag properly labeled with names. The bagged beddings in preschool classroom 1 ,2 and 3 have it in separate big ziploc bags and placed in bins. LPA took photos of the observed bins and cubbies. Also, on 8/16/2023, LPA Chan interviewed staff about staff yelling at daycare children and staff handling children in a rough manner. All 5 staff have similar responses when asked about the above allegations. Staff 1 (S1) stated that S1 have not witnessed any staff being rough at a child and they model good behavior to the class. Staff 2 (S2) Stated she did not witness any staff being rough at a child and they teach children “nice touches” when a child is showing behavior. Staff 3 (S3) stated that S3 did not witness any staff being rough or yell at anyone. S3 stated they redirect children when children are showing challenging behaviors. Staff 4 (S4) stated that S4 never witnessed a staff be rough or yell at a child. Staff 5 (S5) stated that S5 did not witness other staff yelling or be rough at any child. On 8/31/23 and 9/6/23 LPA attempted to do a phone interview with children’s authorized representatives and 3 out of 6 adults responded. Adult 1 (A1) stated they have no complaints about the facility and facility does a good communication. Adult 2 (A2) stated teachers are very active with the children and that her child became attached to the teachers. Adult 3 (A3) stated they are satisfied with the care that the facility provides. Page 2 of 3 Based on observation and interviews conducted, the complaint alleging Staff are not ensuring that children’s bedding does not come into contact with other children’s bedding (2) Staff yells at day care children (3) Staff handle day care children in a rough manner are found to be UNSUBSTANTIATED. Although the allegations may have happened or is valid, there is not enough preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegations are unsubstantiated. Exit interview was conducted. Notice of Site Visit was posted during the visit. Assistant Director was informed that the notice of site visit must be posted for 30 consecutive days. Failure to post will result in civil penalties of $100. Director was provided a copy of their appeal rights (LIC 9058 01/16) and their signature on this form acknowledges receipt of these rights. page 3 of 3
