What the inspector wrote

On 12/27/2024, at 11:35am Licensing Program Analyst (LPA), Anna Chan conducted Case Management inspection Upon arrival, LPA met with Director Lilana Gurrola, LPA informed director of the purpose of visit. LPA went on a walkthrough of the facility and a census was taken. LPA observed 2 staff and 5 infant children. A review of the Facility Personnel Report Summary shows all facility staff or individuals who require caregiver background checks have received a criminal record clearance and a child abuse index clearance or an exemption clearance. During a licensing visit, LPA discovered that staff did not notify the Department of a communicable disease outbreak in the facility during the week of December 17, 2024. LPA interviewed the director, director stated there were atleast 3 cases of a communicable disease outbreak (conjunctivitis) and did not notify the Department via telephone, email or mail as stated in the reporting requirements regulation. The facility was not in compliance with the California Code of Regulations, Title 22, Division 12, Section 101212(d)(1)(C) Reporting Requirements. Please refer to LIC809D for details of deficiency. Director was provided a copy of Reporting Requirements regulation. An exit interview was conducted with Director, Liliana Gurrola. The Notice of Site Visit was posted during the visit. The director was informed that the Notice of Site Visit must be posted for 30 consecutive days. Failure to post will result in civil penalties of $100. The director was provided a copy of their appeal rights (LIC 9058 01/16) and their signature on this form acknowledges receipt of these rights. First-level appeals should be sent to the regional manager to the address listed above. End of Report

This poses as a potential risk to health, safely of children in care.
