Licensing Program Analyst (LPA) Patricia Duron conducted an unannounced case management investigation for the incident which the facility self reported to licensing office on 09/20/23. The unusual incident report stated an unusual incident occurred at the facility on 9/19/23, the Director stated Child #1 (C1) and Staff #1 (S1) were in classroom, a parent (P1) heard yelling and heard a staff saying "stop crying". Director stated that P1 stated they witnessed S1 shaking C1's body and head. Upon arrival LPA, was met by Director Grace Kang, LPA explained the reason for today's visit. Director provided a tour of the facility. LPA observed 5 preschool staff and 38 preschool children in care during today’s visit. A review of facility Personnel Report Summary on this date indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. During today's inspection, LPA Duron interviewed 5 staff, 3 children and review files (children and staff) LPA obtained facility roster. Due to insufficient information available at this time, case management needs further investigations. An exit interview conducted with Director. The Director was provided a copy of appeal rights (LIC 9058 ) and their signature on this form acknowledges receipt of these rights. All appeals must be in writing and received by the Regional Office within 15 business days. Notice of Site Visit was provided and posted. Notice of site visit must be posted for 30 consecutive days. Failure to post will result in civil penalties of $100.00.
