What the inspector wrote

On 11/21/2025, Licensing Program Analyst (LPA) Deschampe conducted an unannounced Plan of Correction (POC) insp ection in response to the Type B violation issued on 09/09/2025: Health and Safety Code (HSC) Section 1597.16(a)(1) A licensed child day care center, as defined in Section 1596.76, that is located in a building that was constructed before January 1, 2010, shall have its drinking water tested for lead contamination levels on or after January 1, 2020, but no later than January 1, 2023, and every five years after the date of the initial test. LPA was met by facility representative (FR), Ivek Rodrigo who allowed LPA entry to the facility at 1:00 PM. LPA explained the purpose of the visit and the FR guided LPA on a tour of the facility with the following census: 13 napping children with 3 staff. During the inspection it was determined the facility is operating within its licensed capacity and within compliance of staffing ratios. A review of the Facility Personnel Report Summary on this date, 11/21/2025 indicates all facility staff, or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. The deficiency that was cited on LIC809-D 09/09/2025 has been cleared through LPA observation, interview and record review. The facility water fixtures and faucets were tested by Environmental Assistance Group per reports dated 10/14/2025 and 11/06/2025. LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP . Drinking water and food preparation will be sourced from the kitchen/break room water faucet with the 5 Stage Reverse Osmosis EconoLine (installed on 11/15/2025). Exit interview conducted and report was reviewed with the facility representative, Ivek Rodrigo. The notice of site visit was given and must remain posted for 30 days. The facility representative was provided a copy of the appeal rights (LIC 9058 ) and their signature on this form acknowledges receipt of these rights. All appeals must be in writing and received by the Regional Office within 15 business days.
