What the inspector wrote

An unannounced case management inspection was conducted on 01/28/2026 by Licensing Program Analyst (LPA), Deschampe. LPA arrived at the facility at 9:15 AM, to complete a Case Management - Incident inspection initiated on 09/24/2025 in response to a self reported Unusual Incident Report (UIR) dated 09/18/2025. LPA was greeted by Dilusha Weerasinghe who introduced herself as the Assistant Director/Teacher. Total enrollment is 21 children with a current census of 15 children with 2 staff, as a result of part-time enrollment. A review of staff criminal records, on this date, 01/28/2026, indicates all facility staff or individuals who require caregiver background checks have received a criminal record clearance or exemption and a child abuse index clearance. On 09/18/2025, the UIR was filed with the Regional Office (RO) to self-report an incident that allegedly occurred on 09/17/2025. The director stated on the report that Child 1 (C1) was allegedly roughly handled by Staff 1 (S1). During the investigation, LPA made observations, interviewed 4 staff, and obtained admission records for C1. C1 did not qualify for an interview based on the Ten Step Investigative Interview questions. LPA left voicemails for 2 parents/authorized representatives and did not receive a call back. During staff interviews, 4 out of 4 staff stated they have never witnessed rough handling between S1 and C1. No deficiencies cited during this visit. The facility representative (FR) was provided a copy of their appeal rights (LIC9058) and their signature on this form acknowledges receipt of these rights. All appeals must be in writing and received by the RO within 15 business days. The "Notice of Site Visit" was posted and must remain posted for 30 consecutive days.
