What the inspector wrote

In conjunction with another inspection on 01/28/2026, Licensing Program Analyst (LPA) Deschampe arrived at the facility at 9:15 AM. LPA was allowed entry to the facility by Dilusha Weerasinghe, who identified herself as the Teacher/Assistant Director. LPA explained the purpose for the case management-deficiencies inspection. Total enrollment is 21 children with a current census of 15 children with 3 staff, as a result of part-time enrollment. A review of staff criminal records, on this date, 01/28/2026, indicates all facility staff or individuals who require caregiver background checks have received a criminal record clearance or exemption and a child abuse index clearance. During today's inspections, LPA observed Staff 1 (S1) use harsh tone and language while speaking with children in care. LPA interviewed staff who confirmed S1 uses stern language and yells at children. S1 and Staff 2 (S2) did not have a complete file for LPA to review. The outdoor area has been divided into a preschool and infant/toddler play area. 6 out of 15 children in care did not qualify using the Ten Step Investigative questions. After the inspection, the following deficiencies were observed and being cited on the attached LIC809-D in accordance with California Code of Regulations (CCR) Title 22 Division 12 Chapter 1: CCR Section 101223(a)(3) Personal Rights CCR Section 101212(b) Reporting Requirements CCR Se ction 101212(c) Reporting Requirements CCR Section 101217(a) Personnel Records Exit interview conducted and report was reviewed with the facility representative, Ivek Rodrigo. Appeal Rights were discussed with the facility representative. The facility representative was provided a copy of the appeal rights (LIC 9058 ) and their signature on this form acknowledges receipt of these rights. All appeals must be in writing and received by the Regional Office within 15 business days. A notice of site visit was given and must remain posted for 30 consecutive days. The “Notice of Site Visit” must be posted on, or immediately adjacent to, the interior side of the main door to the facility. Failure to post will result in Civil Penalties of $100.00.
