What the inspector wrote

On 04/08/25 at 8:55am Licensing Program Analyst (LPA), Anna Chan conducted an unannounced onsite visit for the purpose of an Annual/Random visit. LPA met with Licensee/Director Vikram Gursahani. LPA was led on a tour of the facility. The inside and outside and the floor and yard plan were verified. Census was taken in individual classrooms. The overall census observed was 5 infants and 4 staff. Infants are doing indoor activity when LPA arrived. During the inspection it was determined the facility is operating within its licensed capacity and within compliance of staffing ratios. Facility hours are 6:30a.m.-6:00p.m., Monday through Friday. A review of the Facility Personnel Report Summary on this date indicates all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. During the inspection of the indoor activity space, items which could pose a danger to children (detergents, cleaning compounds, and medications) were observed to be stored out of the reach of children. Age-appropriate materials are provided. Facility does not provide food; food is brought from home. Food prep areas were clean and sanitary. Floors, equipment, and furniture were clean and were observed to be in good repair and free of sharp edges. There is drinking water available to children indoors by Brita water pitcher and sippy cups are also brought by children from home with the child’s name on them. Page 1 of 4 Younger infants aged 0 to 12 months nap on cribs and older infants 12 months and above nap on cots, and bedding is stored individually. Facility provides sheets which is washed by the facility weekly. Blankets are brought home at the last day of child in school for washing. The facility has a working carbon monoxide detector and fire extinguisher. The facility has conducted an emergency fire drill on December 11, 2024. Facility met all posting requirement. The California Child Passenger Safety Law was posted by the entrance of the facility. The outdoor activity space was inspected for compliance. The playground was enclosed by a fence at least four feet in height. The surface of the outdoor activity space was well maintained and free of hazards. The cushioning material is turf grass and there is a big sand box where the climbing equipment are and appeared to be enough to absorb falls. Drinking water in the outdoor activity space is provided by sippy cups with the child’s name on it. The outdoor equipment and toys were in good repair and free of sharp edges. There are no bodies of water present at the facility. The facility grounds were safe, sanitary and in good repair. Staff files were reviewed for staff present during the facility inspection on this date, 4 staff files were reviewed. Health screening and immunization as required were reviewed. Beginning September 1, 2016, Health and Safety (H&S) 1596.7995 (a)(1) a person shall not be employed or volunteer at a day care center if he or she has not been immunized against influenza, pertussis, and measles. Each employee and volunteer shall receive an influenza vaccination between August 1 and December 1 of each year. Proof of immunization against pertussis, measles for the facility were reviewed and within compliance. Beginning March 31, 2018, H&S Code 1596.8662 requires all directors and employees to complete mandated reporter training, and to renew the training every two years. At least one staff member present possesses current EMSA approved Pediatric CPR/First Aid certifications which expires 08/2025. Page 2 of 4 5 Children's records were reviewed, and there was a separate, complete, and current record for each child. Infant files do have LIC9227 or Individual Infant Sleeping Plan and a 15-minute infant sleep log. Sign in/out procedure in LILLIO app was reviewed, each child representative has their own login. In the areas reviewed, children’s files were in compliance. Currently, there are medications in the facility. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated. Transfer Request (LIC9182). An Inspection and exit interview were completed with Director Vikram Gursahani. The report was reviewed and discussed. Appeal Rights was provided. The “Notice of Site Visit” was posted on or adjacent to the door. Notice of Site Visit” must be posted for 30 consecutive days. Page 4 of 4
