What the inspector wrote

Page 2 During the course of the investigation LPA interviewed 4 staff and 7 parents on 07/02/21 and 08/12/21. LPA also reviewed children\342\200\231s roster, staff roster, sign-in / sign-out sheets for staff and children, obtained copies of subject child\342\200\231s file from this facility, and obtained Orange County Child Protective Services (CPS) report. LPA made attempts to interview the RP via phone, LPA did not receive a call back. LPA called subject child\342\200\231s parents; LPA did not receive a call back. Children were not interviewed due to being non-verbal due to age. There was no CPS report for the allegation dated 06/29/21 regarding \342\200\234crusty poop\342\200\235 on subject child\342\200\231s rear end. There was a CPS report for the allegation on 06/09/21, the disposition on the report states \342\200\234info only\342\200\235, CPS did not investigate the allegation. Based on records reviewed, the subject child was dropped off to this childcare facility on 06/08/21 at 8:30am by subject child\342\200\231s mom, child was fine, no swelling on child\342\200\231s private parts. Subject child was picked up by the subject child\342\200\231s dad on 06/08/21 at approximately 4:00pm from this childcare facility. The subject child was in the father\342\200\231s care when the discovery was made the following day 06/09/21 at 8am where a string of hair was discovered wrapped around subject child\342\200\231s private part. Subject child was taken in to be seen at a hospital by a specialist on 06/09/21. Records reviewed state the color of the hair did not match that of the director\342\200\231s but did match the color of hair for one staff and of the subject child\342\200\231s mom. LPA interviewed director and 3 staff who stated diapers are changed in the morning before children\342\200\231s first snack around 10am, then after lunch (lunch is around 12:00pm until when children finish their food), then again after nap (nap is between 1:00pm-3:00pm depending on when children wake up), and if the child is at the facility late diapers are changed again around 4pm. All 4 staff stated they check in between those intervals stated and will change diapers if needed. All 4 staff describe the process of cleaning a child with using as much baby wipes as possible until the child is thoroughly cleaned. LPA interviewed director who stated subject was last at the facility on 06/29/21, sign-in/sign-out sheet confirmed this information and indicate subject child was picked up at approximately 4:50pm by Adult #1, A1 (See Confidential Names List dated 08/24/21). Director states she received a call from A1 at approximately 7:41pm (3 hours after pick up) 06/29/21, A1 was asking about diaper changing. Director informed A1 that subject child\342\200\231s diaper was changed before subject child was picked up. Director asked A1 if this is the first time this had happened, A1 responded yes. Director asked \342\200\234are you guys okay with everything at Kiddieland?\342\200\235, A1 responded yes. (Continued on page 3) Page 3 No other concerns were brought up by A1 per Director. Director states at approximately 4:10pm she personally walked subject child to her staff S1 (See Confidential Names List dated 08/24/21) to change subject child\342\200\231s diaper. S1 confirmed the information but approximates the time between 3:30pm-3:45pm. All 4 staff interviewed state they would have known by smell if subject child had poop in the child\342\200\231s diaper as staff walk children out to their parents during pick up. All 4 staff add that children\342\200\231s diapers are checked before leaving the facility. LPA interviewed 7 parents who stated they were satisfied with the care provided by the staff at this facility, parents did not bring up any concerns. Parents state their children are happy and like coming to this facility. Parents state they would recommend this facility to other parents. In addition, LPA received 8 emails from parents of children who are cared for or were cared for at this facility describing how satisfied they were with the care provided by staff at this facility. Based on interviews conducted and records reviewed there is not enough evidence to support the allegations. According to staff interviewed child\342\200\231s last diaper change at this facility was between 3:30pm \342\200\223 4:10pm, child was picked up at 4:50pm and A1 called director at 7:41pm, approximately 3 hours later. There is a 4 hour time frame between 3:30pm to 7:41pm where the child was either with staff at this facility or home. Regarding the second allegation which happened 06/09/21, director was unaware because no concerns were brought up by either of the subject child\342\200\231s parents or by A1. Parents of subject child continued to take subject child to this facility after 06/09/21 until subject child\342\200\231s last day on 06/29/21. Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegations are unsubstantiated. An exit interview was completed with director. The report was reviewed and discussed. Appeal Rights (LIC 9058) were discussed and a copy of the Appeal Rights was provided, their signature on this form acknowledges receipt of these rights. All appeals must be in writing and received by the Regional Office within 15 business days. A Notice of Site Visit was provided. The director was informed that the \342\200\234Notice of Site Visit\342\200\235 must be posted for 30 consecutive days on or adjacent to the door. Failure to post will result in Civil Penalties of $100.00.
