(Page 1) Licensing Program Analyst (LPA) Giselle Lucero conducted a case management inspection of the facility for the purpose of room change. LPA met with Lead Teacher Rubid Ortiz who gave a tour of the facility to LPA. Currently the facility is licensed for 24 preschool children in Room 9. LPA observed there were 3 preschool staff with 15 preschool children. A review of the Facility Personnel Report Summary on this date indicates all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. (The staff are exempt of this requirement due to staff being employed by Anaheim School District and they are fingerprint cleared under the school district). The facility has requested to provide care and supervision to 24 preschool children in room 51. Monday to Friday from 7:30 A.M to 4:00 P.M. Room 51 measured a total of 963.97 square feet which is sufficient for the requested capacity. There are 2 sinks and 2 toilets for children\342\200\231s use which is sufficient for the requested capacity. Room 51 has age appropriate toys and equipment. The Anaheim fire authority approved the facility for maximum of 24 preschool children in room 51 approved on 12/19/2024. (Continue to page 2) (Page 2) Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platform. Exit interview conducted and report was reviewed with Lead Teacher Rubid Ortiz. A notice of site visit was given and must remain posted for 30 days. Based on today\342\200\231s indoor measurements, the sinks and toilets available, the request of the facility to utilize room 51 is granted effective today\342\200\231s date. End of report
