At 9:03 AM, Licensing Program Analyst (LPA) Deschampe conducted a Plan of Correction (POC) inspection in response to the Type B violation issued on 11/04/2025 California Code of Regulations (CCR) 101238.2(d)(2) The surface of the outdoor activity space shall be maintained: Free of hazards including, but not limited to, holes, broken glass and other debris, and dry grasses that pose a fire hazard . LPA met with Facility Representative/Assistant Director (FR), Kristine Fernandez. During the inspection there was 30 children with 3 staff. A review of the Facility Personnel Report Summary on this date, 12/09/2025 indicates all facility staff and other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. On 12/09/2025, LPA observed the missing pieces of privacy perimeter fence and broken pole of the perimeter fence have been addressed with the use of a mesh covering. All broken toys/equipment have been removed. LPA received documentation of the monthly maintenance log/checklist. The deficiency that was previously cited on a LIC 809-D dated 11/04/2025 has been cleared. No further action needed at this time. POC letter given to FR and correction has been received at this time. Exit interview conducted and report was reviewed with the facility representative/assistant director, Kristine Fernandez. Appeal rights were discussed with facility representative. The facility representative was provided a copy of the appeal rights (LIC 9058 ) and their signature on this form acknowledges receipt of these rights. All appeals must be in writing and received by the Regional Office within 15 business days. A notice of site visit was given and must remain posted for 30 consecutive days. The \342\200\234Notice of Site Visit\342\200\235 must be posted on, or immediately adjacent to, the interior side of the main door to the facility. Failure to post will result in Civil Penalties of $100.00. End of report.
