What the inspector wrote

On 11/04/2025, Licensing Program Analyst (LPA) Deschampe conducted an unannounced Plan of Correction visit to follow up on the Type B deficiencies issued on 10/21/2025: California Code of Regulations (CCR) Infant Care Center Sleeping Equipment 101439.1(b)(1) A crib or portable -crib meeting United States Consumer Product Safety Commission safety standards shall be provided for each infant who is unable to climb out of a crib (1) Mattresses shall be firm and covered with a fitted sheet that is appropriate to the mattress size, fits tightly on the mattress, and overlaps the underside of the mattress so it cannot be dislodged, CCR Responsibility for Providing Care and Supervision for Infants 101429(a)(2)(C) Documentation shall be maintained in the infant’s file and be available to the Department for review, and Health and Safety Code Administration of Child Day Care Licensing 1596.8662(b)(1) On or before March 30, 2018, a person who, on January 1, 2018, is a licensed child day care provider, administrator, or employee of a licensed child day care facility...shall complete renewal mandated reporter training every two years following the date on which he or she completed the initial mandated reporter training. LPA arrived at the facility at 9:30 AM and was met by the licensee, Mary Chang. Licensee guided the LPA on a tour of the inside and outside of the facility with the following census: 19 infants with 6 staff. During the inspection it was determined the facility is operating within its licensed capacity and within compliance of staffing ratios. A review of the Facility Personnel Report Summary on this date indicates all facility staff, or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. Facility hours are Monday through Friday 6:30 AM-6:00 PM. LPA confirmed the licensee, Mary Chang has corrected the violations. Through LPA's observations and record review during this visit, the violations which were cited on 10/21/2025, are cleared. LPA observed the infant safe sleep 15 minute check documentation were completed and filed for all infants. LPA observed 10 out of 10 infant cribs had a fitted sheet that is appropriate to the mattress size, fits tightly on the mattress, and overlaps the underside of the mattress so it cannot be dislodged. LPA observed through record review, 8 out of 8 infant staff have completed their mandated reporter training. The exit interview was conducted and report was reviewed with the licensee, Mary Chang. Technical Support Program (TSP) referral discussed and accepted by licensee. The notice of site visit was given and posted during the inspection. Licensee was reminded that the notice of site visit must be posted for 30 consecutive days. Appeal rights were discussed and provided to the licensee and their signature on this form acknowledges receipt of these rights. Licensee must file an appeal, in writing, to the Regional Manager within 15 business days from the date of receiving the penalty assessment or notice of deficiency. First level appeals should be sent to the Regional Manager at the address listed above. End of Report
