What the inspector wrote

On 12/17/2025, Licensing Program Analyst (LPA) Deschampe conducted an unannounced Plan of Correction inspection to follow up on the Type A deficiency issued on 12/09/2025. At 9:10 AM, LPA entered the facility. LPA was met by met by the teacher, Leann Jacob. LPA met with Facility Representative, Kristine Fernandez and explained the purpose of the visit. Total census was 19 infants with 6 staff. A review of the Facility Personnel Report Summary on this date, 12/17/2025 indicates all facility staff and other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. California Code of Regulations (CCR) Section 101429 Responsibility for Providing Care and Supervision for Infants (a) In addition to Section 101229, the following shall apply: (1) Each infant shall be constantly supervised and under direct visual observation and supervision by a staff person at all times. LPA confirmed with the facility representative, Kristine Fernandez the child care center has corrected the violation. Based on LPA's observations, interviews, and records review during this visit, the violation which was cited on 12/09/2025, is cleared. Facility representative stated infant staff will maintain small groups during outdoor play and will complete ratio counting, regularly. LPA observed, Acknowledgement of Receipt of Licensing Reports (LIC 9224) was signed by parent/authorized representative in 24 out of 24 enrolled children. The exit interview was conducted and report was reviewed with the facility representative, Kristine Fernandez. The notice of site visit was given and posted during the inspection. Facility representative was reminded that the notice of site visit must be posted for 30 consecutive days. Appeal rights were discussed and provided to the facility representative and their signature on this form acknowledges receipt of these rights. Licensee must file an appeal, in writing, to the Regional Manager within 15 business days from the date of receiving the penalty assessment or notice of deficiency. First level appeals should be sent to the Regional Manager at the address listed above. End of Report.
