On 01/27/2026, Licensing Program Analyst (LPA) Deschampe conducted an unannounced Plan of Correction inspection to follow up on the Type B deficiency issued on 10/22/2025. At 8:39 AM, LPA entered the facility and met with Facility Representative (FR), Kristine Fernandez and explained the purpose of the visit. Total enrollment is 24 children, with a census of 20 infants and 6 staff, census due to various family schedules. A review of the Facility Personnel Report Summary on this date, 01/27/2026 indicates all facility staff and other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. California Code of Regulations (CCR) 101238.2(d)(2) The surface of the outdoor activity space shall be maintained: (2) Free of hazards including, but not limited to, holes, broken glass and other debris,... The facility emailed pictures of the newly installed turf in the outdoor infant/toddler play area. The FR shall use blankets when the turf is hot to touch and will continue maintenance of turf, every weekend. Based on LPA's observations, interviews, and record review during this visit, the violation which was cited on 10/22/2025, is cleared. The exit interview was conducted and report was reviewed with the facility representative, Kristine Fernandez. The notice of site visit was given and posted during the inspection. Facility representative was reminded that the notice of site visit must be posted for 30 consecutive days. Appeal rights were discussed and provided to the facility representative and their signature on this form acknowledges receipt of these rights. Licensee must file an appeal, in writing, to the Regional Manager within 15 business days from the date of receiving the penalty assessment or notice of deficiency.
