What the inspector wrote

Page 2 On 11/02/2021, LPA conducted an interview with 4 staff including the director. According to the director, the child was absent for a good part of October due to not wanting to attend. Based on the attendance, C1 was present at school on 10/1/21, 10/4/21, 10/5/21, 10/6/21 and 10/12/21. On October 27, 2021, the parent came to pick up C1\342\200\231s belongings, and informed director C1 will be attending the Halloween event the next day on October 28, 2021. However, parent had just informed the director the child was still sick, and director asked for the child not to attend. The number of children enrolled on October 28, 2021 were 45 preschool and 11 staff. LPA inquired about reservations for party and director confirmed no reservations needed. No children were interviewed due to Covid restrictions and allegations did not pertain to children. On 11/01/2021 LPA interviewed complainant who stated C1 did not attend school from October 18-22 due from being sick. The complainant also stated there was no communication with the school about C1 absence or days C1 would attend. On 12/01/21, LPA called 11 parents, LPA was able to interview 5 of the parents, none made any disclosures. LPA did not receive a call back from the 6 parents. This agency has investigated the complaint alleging facility refused to accept child on regular scheduled day. Although the allegation may have happened or is valid, there is not enough preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is unsubstantiated. Exit interview was conducted. Notice of Site Visit was posted during the visit. Director was informed that the notice of site visit must be posted for 30 consecutive days. Failure to post will result in civil penalties of $100. Director was provided a copy of their appeal rights (LIC 9058 01/16) and their signature on this form acknowledges receipt of these rights. First level appeals should be sent to the regional manager to the address listed above.
