*** This is Amended report to correct the date of the incident on the 809 report that was provided 3/10/2025. *** Licensing Program Analyst (LPA) Trinh conducted an on-site inspection for the purpose of a self reported incident that occurred on 03/3/2025. The Director Patricia Abeyta, was informed of the visit’s purpose and a tour was completed and tour LPA around the center. The census was 75 children, and 13 Staffs members including Director. A review of the Facility Personnel Report Summary conducted on today’s date 03/10/2025 indicates all facility staff or other individuals who required caregiver background checks have received criminal record and child abuse index clearances or exemptions. On 03/4/2025 an Unusual Incident Report was filed with the Department to self-report an incident that occurred on 03/4/2025, Child #1 (C1) got hurt by another Child #2 (C2) on the playground. During the course of the investigation, LPA interviewed Center Director Patricia Abeyta , and 6 staff members, 3 children, obtained a copy of the children's roster (LIC 9040), Child #1 incident report to parents, facility roster, sign-in/out copy of the day of the incident, and C1’s and C2’s records. Due to additional information needed, the report incident needs further investigation. An exit interview was conducted. The Notice of Site Visit was posted for at least 30 consecutive days. Appeal Rights were explained. A copy of appeal rights (LIC 9058) was provided, and their signatures on this form acknowledged receipt of these rights. The End of Report.
