On 08/26/2025 Licensing Program Analyst (LPA) K. Navar conducted an unannounced case management inspection for the purpose of a follow up inspection that was completed on 6/302025. LPA met with director Cathy, Sipia census was taken and there were 0 school age present and 20 preschool children and 3 staff during nap time. Children were resting on cots and staff were supervising. DIrector stated school age children will arrive about 2:30/3:00PM. On 6/30/2025 LPA Navar conducted an annual inspection, and facility was out of compliance based on observations and interview S3 stated that staff and children share the bathroom located in the preschool hallway and that adults do not use it at the same time as children do which poses/posed a potential health, safety or personal rights risk to persons in care. During today\342\200\231s inspection on 8/26/2025 LPA observed a total of 4 sinks, 3 toilets, and 1 urinal available for children\342\200\231s use. S4 informed LPA that a staff bathroom is available outside of the facility in the office building for staff to use. LPA verified and observed staff bathroom and location. On this date director stated S4 is not left alone and is in the preschool program. In the areas that were evaluated, facility was in compliance with Title 22.
