What the inspector wrote

Page 2 During the investigation LPA Odom interviewed director and 6 staff members. LPA conducted a facility inspection in each preschool classroom, reviewed the children\342\200\231s roster, personnel report, pest service agreement, janitorial service agreement, and weekly cleaning schedule. Children and parents were not interviewed due to enough evident gathered. During the investigation, Director (S1) was interviewed on 09/22/22. S1 stated they have observed children coming to the preschool with mosquito bites. S1 stated they have a pest service agreement with The Bugman who comes to the childcare monthly to spray the inside and outside of the childcare and power wash the outside for insects as well. The pest company has not mentioned to S1 regarding any issues with infestation with insects. S1 stated they have a janitor that comes to the childcare every evening to clean the childcare and they have not mentioned to S1 regarding any issues with insects. The staff have a weekly cleaning schedule that they turn in to S1 and staff have not brought up any issues with insects. S1 disclosed parents have not complained about insect bites. During the investigation, 6 staff members were interviewed on 09/22/22. Three out of six staff stated occasionally they have observed bites on children, and they have been bit themselves in the childcare. They believe these to be mosquito bites gotten when staff and children are outdoors, and at times indoors. None of the staff have addressed the mosquito bites with S1 as a concern. None of the staff disclosed parents being concerned with the bug bites. All of the staff stated they clean their classroom multiple times a day, and a janitor will come every day to clean the childcare center. Based on LPA\342\200\231s facility inspection, observations, interviews conducted with director, 6 staff members and records reviewed it was determined there was insufficient evidence that facility has insect. Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegations are Unsubstantiated. Exit interview was conducted with Director Kristi Bernhardt. Notice of Site Visit was posted during the visit. Director was informed that the notice of site visit must be posted for 30 consecutive days. Failure to post will result in civil penalties of $100. Director was provided a copy of their appeal rights (LIC 9058 01/16) and their signature on this form acknowledges receipt of these rights. First level appeals should be sent to the regional manager to the address listed above.
