What the inspector wrote

On 01/23/26, A case management inspection was conducted today by Licensing Program Analyst (LPA) Anna Chan who met with Director Lori Macias and owner Samantha Madrigal and was informed of the reason of visit. Upon arrival there were 24 infants and 8 staff doing indoor activities. Owner stated there are 31 infants enrolled in the program. The purpose for today's inspection was for LPA to review Safe Sleep procedures including the use of Sleep Sacks. LPA reviewed with the director and owner the following safe sleep best practices: \302\267 Always place infants on their backs for sleeping \302\267 Use only a tight-fitting sheet on the crib or play yard mattress \302\267 There shall be no objects hanging above or attached to the side of the crib. \302\267 Keep all items, including blankets, out of the crib or play yard \302\267 Pacifiers may be used if they do not have items attached to them \302\267 Infants should not be swaddled or have any items covering them while sleeping \302\267 The temperature of the room should be comfortable enough for an adult to wear a t-shirt and not be too hot or too cold. LPA discussed safe sleep regulations 101430 Infant Care Activities and 101429 Responsibility for Providing Care and Supervision for Infants with the facility and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. A sample of every 15-minute check on children under 24 months old during nap time for logging was given to the facility. page 1 of 2 Safe Sleep FAQ was provided. The FAQ states that sleep sacks are a form of swaddling therefore require an exception. Director and owner was reminded that the use of sleep sacks as a swaddling alternative would require an exception and shall be considered on a case-by-case basis. Director and owner stated they will discontinue use of sleep slacks effective 1/23/26. Director and owner stated they understand that a request for an exception should be submitted to the Department to be reviewed on a case-by-case basis. LPA observed 2 infants napping and not wearing sleep sacks. LPA observed command hooks attached in one side of the crib, director stated the command hooks are being removed. Owner Samantha Madrigal is requesting for a meeting with management to get detailed information about sleep sacks No deficiencies cited during this visit. If any deficiencies are observed, disclosed or discovered, they will be addressed and cited at a later date. Exit interview was conducted with Director Lori Macias. The Notice of Site Visit was posted. Director was informed that the Notice of Site Visit must be posted for 30 consecutive days. Appeal Rights were provided. End of Report
