What the inspector wrote

On 2/06/2025 Licensing Program Analysts (LPAs) Karen Navar and Susan Deschampe conducted a Case Management Inspection to investigate previous deficiency which was cited during an investigation dated 2/03/2025. LPA Duron informed the director of the purpose of the visit. LPA met with Director, Nicole Carreon. LPA observed 31 infants and 9 staff, 60 preschool children with 5 staff members. During the inspection it was determined the facility is operating within its licensed capacity and within compliance of staffing ratios. A review of the Facility Personnel Report Summary on this date indicates all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. On 2/03/2024 facility was cited, California Code of Regulations, Title 22, Division 12, Chapter 1 Section 101223(a)(3) Personal Rights(a) The licensee shall ensure that each child is accorded the following personal rights: (3) To be free from corporal or unusual punishment, infliction of pain, humiliation, intimidationt…Based on records review and staff interviews, LPA observed video footage and Unusual Incident Report (UIR) of S14 forcibly laying C2 on cot. 2 (two) staff interviews stated S14 forcibly laid C2 on their cot. Type A citation was given. LPAs Navar and Deschampe conducted this inspection to review facility’s procedures to ensure facility is following California Code of Regulations, Title 22 Regulations, Division 12. During inspection LPA took census and observed children’s files for LIC9224 Acknowledging Licensing Reports. LPAs observed 6 LIC9224 forms had been signed since 2/03/2025. Based on LPAs record reviews the following violation was observed in accordance with California Code of Regulations, Health and Safety Code-HSC, Section 1596.8595(c)(1) , is being cited on the attached LIC 809D. Continue to page 2. Page 2 Exit interview was conducted with Director Nicole Carreon. LPAs reviewed HSC 1596.8595 Posting licensing report by childcare facility or home; duration of posting; civil penalty for failure to comply; reports to be provided to parents or guardian of each child receiving services. The facility representative was informed that the “Notice of Site Visit” must be posted for 30 consecutive days. The “Notice of Site Visit” must be posted on or adjacent to the door. Failure to post will result in Civil Penalties of $100.00.
