What the inspector wrote

On 05/6/2025 Licensing Program Analyst (LPA) K. Navar conducted an unannounced Case Management – Incident visit to investigate an unusual incident. Upon arrival, the LPA met with Director Nicole, Carreon and Loretta, Munoz and informed them of the purpose of the visit. A review of the Facility Personnel Report Summary shows all facility staff or individuals who require caregiver background checks have received a criminal record clearance and a child abuse index clearance or an exemption clearance. The census at the time of the visit was 76 children supervised by five (5) staff during nap time. Children were resting/sleeping on their beds. The Department received an incident report on 04/28/2025 stating that admin had a meeting with parent 1 (P1) who had stated staff 1 (S1) had alleged misconduct with C1. During the investigation, LPA toured the facility with Director Munoz and conducted three (3) staff interviews (S1-S3) and obtained copies of staff 1’s (S1) timecard report and work schedule. 3 staff interviews stated that S1 is never left alone with children and supports staff in morning care. Based on the LPA’s observations, interviews, and records review, if any deficiencies are observed, disclosed, or discovered during today’s visit, they will be addressed and cited at a later date. An exit interview was conducted with Director Nicole, Carreon. The Notice of Site Visit was posted during the visit. The director was informed that the Notice of Site Visit must be posted for 30 consecutive days. Failure to post will result in civil penalties of $100. End of Report.
