Follow-up on corrections·December 18, 2025

On 12/18/2025 at Licensing Program Analysts (LPAs) Olivia Meza and LPA Anna Chan conducted a case management visit. Upon arrival, LPA Meza and LPA Chan met with the Director Loretta Munoz and informed the Director the purpose of the visit. LPAs and Director toured the facility. There were 56 preschool students, and nine (9) staff present. During staff record review it was discovered that Staff 1 (S1) did not have a criminal record clearance. LPAs reviewed guardian with the Operation Manager Melissa Wigton and verified that the staff member was not cleared. LPAs provided guardian notifications of incomplete fingerprint. Facility was reminded that individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. Director was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed. LPA Meza informed Director, Loretta Munoz that this report dated 12/18/25 documents Type A citations which shall be posted for 30 consecutive days as there is immediate risk to the health, safety, or personal rights of children in care. Civil Penalties are being assessed, see LIC809D for deficiencies. LPA Meza also informed the Director, Loretta Munoz to provide a copy of this licensing report dated 12/18/25 that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. Exit interview conducted and report was reviewed with the Director, Loretta Munoz. A notice of site visit was given and must remain posted for 30 days.
