What the inspector wrote

Licensing Program Analyst (LPA) Cynthia Sun conducted an unannounced case management incident inspection in response to a self-reported Unusual Incident dated 12/09/2025. During the inspection, LPA met with Director, Sirangi Salgado and observed 22 preschool children and 4 staff members. Children were getting ready for nap while staff supervised children. A review of staff criminal clearance records on this date indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. On 12/10/2025, the facility submitted a self-reported Unusual Incident Report (UIR) to the Licensing Office regarding an incident that occurred on 12/09/2025. The report stated that Child #1 (C1), was terminated from the program after director met with C1’s parent (P1). During the visit, LPA interviewed director who stated that on 12/08/25, director called P1 to come to facility to speak regarding C1’s behavior because C1 was hitting teachers and children. P1 came to facility and talked to director about C1's behavior. After the conversation about C1’s behavior, P1 decided to pull C1 out of program. PAGE 1 of 2 LPA interviewed staff and asked how staff interactions with C1 were. 4 out of 4 staff stated C1 would hit other children and staff without provocation or warning. Staff #4 (S4) stated, “One time, I was taking children to bathroom and C1 kicked me. I told the director”. Staff #3 (S3) stated “C1 pushed me in the classroom and outside in the play yard on 12/8/25. I declined to go to the doctor.” S3 stated the other children were scared of C1. Staff #2 (S2) stated “C1 would often grab me or pull my sweater”. LPA asked staff if staff called to inform P1 regarding C1s behavior. All staff stated they would not call parent. They would communicate incidents with director and director would communicate with C1’s parent. Director provided LPA with a copy of P1 signed facility termination agreement stating facility reserves the right to dismiss any student, if necessary, for the well-being of the school. LPA interviewed P1 on 12/17/25 who stated P1 decided to terminate contract with facility and pulled C1 out of facility because P1 kept getting complaints about C1 hitting children and teachers. LPA asked P1 if facility was communicating with P1 about C1’s behavior. P1 stated “Yes, they would text me and tell me”. Based on LPA’s interviews and record review, the facility was in compliance with regulations, and no deficiency was observed. Exit interview was conducted. The Notice of Site Visit was posted. Program Director Sirangi Salgado was informed that the Notice of Site Visit must be posted for 30 consecutive days. Failure to post will result in civil penalties of $100. PAGE 2 OF 2 END OF REPORT
