What Happened

Case-management review substantiated a supervision lapse when a child remained outside the classroom during a transition and was found at the back door shortly afterward. Staff responded by changing transition procedures and adding self-counting to their name-to-face process.

  • staff changed their transition protocol to keep a child from going behind the door
