What Happened
The annual inspection found the facility operating within capacity and ratios, but LPAs observed an unapproved room addition that had not been reported to the Department in writing. Staff record review also found missing background clearance-related documentation and mandated reporter training issues.
Observations
- A room next to classroom #4 had been added without a city permit and without prior written notice to the Department
- Nina Nguyen and PhuongLoan Le were missing fingerprint clearance at the time of inspection
- Several staff were missing mandated reporter training, and one staff member's training was expired
- One staff member was missing a health screening record

Each personnel record shall contain the following information: (11) A health screening as specified in Section 101216(g).This requirement is not met: Based on record review, S2 is missing health screening. This poses a potential Health and Safety risk to the children in care.
(b)(1)On or before March 30, 2018, a person who, ..licensed child care provider, administrator, ... mandated reporter training provided pursuant to paragraphs (2) and (3) of ... mandated reporter training every two years following the date on which he or she completed the initial mandated reporter training.This requirement is not meet: Base on record review, S1, S2, S3 is missing Mandated reporter training. Also S4's Mandated reporter training was expired on 3/11/2020. This poses a potential Health and Safety risk to the children in care.
101170 Criminal Record Clearance (e) All individuals subject to a criminal record review pursuant to Health and Safety Code Section 1596.871 shall prior to working, residing or volunteering in a licensed facility:(1) Obtain a California clearance...This requirement is not met as evidenced by: Based on record review and interview, Nina Nguyen worked at the facility without criminal clearance since April 26,2021 and PhuongLoan Le worked at the facility without criminal clearance since May3, 2021. The director failed to obtain a california clearance. This poses an immediate Health and Safety risk to the children in care.
