What the inspector wrote

On 05/22/26 at 12:45pm, Licensing Program Analyst (LPA) Anna Chan made an unannounced inspection to the facility for the purpose of a Case Management-Incident visit. LPA met with Assistant Director Simina Osmani and toured the facility. Census was taken. LPA observed 93 children including 12 toddlers and 7 staff. Children were napping when LPA arrived. Director Stephanie Evangelista-Recto arrived shortly after to continue with the visit. A review of criminal record clearances indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. A self-reported incident was received by the Department on 5/18/26 that on 5/7/26 a Parent meeting was conducted at the facility for an alleged Staff hitting a child. LPA interviewed 5 staff and 5 children. LPA obtained documents such as, copy of UIR, Email Correspondents with Family, Admissions Agreement, Staff Roster and Staff Statement Report, Child's Attendance Record including child's incident reports. If any deficiencies are observed, disclosed or discovered during today’s visit, they will be addressed and cited at a later date. Exit interview was conducted with Director Stephanie Evangelista-Recto. Notice of site visit posted at entrance of facility and must remain posted for 30 consecutive days. Appeal Rights were provided. End of Report
