What the inspector wrote

On 12/04/25 at 9:15am, Licensing Program Analyst (LPA) Sarah Garcia conducted an unannounced case management- incident visit in response to a self- reported unusual incident report (LIC 624) received on 12/01/25 regarding a child being left unattended. LPA met with Director, Elizabeth Orizabal. A walk through of the facility was conducted, and a census was taken. Total census was 18 preschool children and 3 staff including director. At 10:15am, LPA met with facility representatives, Elizabeth Cruz and Sylvia Cuesta. On 12/01/25 the facility reported to the Department that a child wandered away from classroom 606 during transition period. A parent located the child near the gate by the preschool entrance and the parking lot. The facility states that the child may have wandered away after following a teacher who exited the facility. The child had been left alone for approximately 2-3 minutes. During today's visit, LPA interviewed 5 staff, obtained incident report (LIC 624), parent handbook, roll call sheet, facility map, staff qualifications, staff statements, Child 1(C1) sign in/out sheet, children's roster. All staff interviewed stated that it was C1's first day at the facility. Between approximately 9:50am-10am, C1 left the classroom through the east or south door. C1 was found outside classroom 606 and near the gate leading to the south parking lot. C1 was found by the English Spanish Language (ESL) students. LPA attempted to speak with Child 1 (C1) but C1 was too young to be interviewed. Continued on Page 2 Page 2 Based on LPAs observations, the area in which C1 was found is approximately 30.6 feet away from the east door entrance and 11ft from the south door entrance. LPA took photographs of the area where C1 was found. Based on record review, Staff 5 (S5) and Staff 6 (S6) witness statements stated they were new staff to the facility. S5 and S6 stated that parents from the ESL class brought in a C1 who wandered away back into the classroom 606. Based on interviews and record review, on 12/1/25 C1 was left unattended outside the classroom for approximately 2-3 minutes. According to Title 22, California Code of Regulations, Section 3, Chapter 1, 101229(a)(1) Responsibility for Providing Care and Supervision, (1) Type A deficiency will be cited today. See attached LIC 809D. LPA Sarah Garcia informed facility representatives, Elizabeth Cruz and Sylvia Cuesta that this report dated 12/4/2025 documents (1) Type A citation which shall be posted for 30 consecutive days as there is and immediate risk(s) to the health, safety, or personal rights of children in care. Also, LPA Sarah Garcia informed facility representatives, Elizabeth Cruz and Sylvia Cuesta, to provide a copy of this licensing report dated 12/4/25 that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. Exit interview conducted and report was reviewed with facility representatives, Elizabeth Cruz and Sylvia Cuesta. A notice of site visit was given and must remain posted for 30 days. Appeal right explained and provided.
