Inspector’s narrative

What the inspector wrote

On 7/28/2025 an unannounced case management inspection was conducted on this date by Licensing Program Analyst (LPA) K. Navar in response to a self-reported incident dated 7/18/2025. Present during today’s inspection was Program Lead-Jocelyn, Ruano. The census was taken and there were 73 school age children and 7 staff. A review of adult records indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. The Department received an incident report on 07/18/2025 stating that on 07/17/25 at 2:07PM that C1 had wrapped a play dough around a pencil and dared C2 to squish the play dough. C2 had accepted the dare which resulted in the pencil puncturing the hand. During the investigation LPA toured the facility on 7/28/2025 with Program Lead and observed staff supervising and engaged with children during different activities. LPA interviewed 3 (three) staff (S1-S3) who were involved with the incident. 2 (two) staff stated that they were supervising children during journal time and that the children who were done journaling were using play dough at the table area and that is when the incident happened and that staff intervened right away and provided first aid, filled an incident report out, reported it to Child Care Licensing, and called parents. LPA obtained a copy of the incident report dated 7/17/2025 and a copy of a BOR-Behavior Observation Report. Based on the information gathered from the interviews with staff and child records reviewed. The facility was in compliance of the California Code of Regulations, Title 22, Division 12 and no citations will be issued. Exit interview conducted and report was reviewed with the Center Manager, Nury, Balmaceda, . A notice of site visit was given and must remain posted for 30 days. Failure to post will result in civil penalties of $100.
