What the inspector wrote

On 12/18/2024, at 3:00pm, a case management inspection was conducted today by Licensing Program Analysts (LPAs) Navar and Deschampe who met with the Program Lead Christina Orellana. A self-reported incident by the facility was received at the regional office on 12/12/24 which stated that facility was out of ratio with 2 staff and 30 children from 2:38PM-2:58PM. Upon arrival there were 3 staff present and 25 children in care. A review of criminal record clearances indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. During this investigation, LPAs toured the facility with Program Lead and interviewed three (3) staff (S1-S3). S2-S3 were present at the time the incident happened. All three (3) staff interviewed stated that facility was out of ratio from 2:38PM-2:58PM with 30 children and that children were participating in homework activities until staff 4 (S4) arrived at 2:58PM. LPAs obtained a copy of the sign in and sign out sheet dated 12/11/2024 and Personnel Summary Report. Based on the information gathered from the interviews conducted and records reviews, it was determined that staff S2 and S3 were out of ratio from 2:38PM-2:58PM with 30 children. The facility was not in compliance of the California Code of Regulations, Title 22, Division 12. The following citation 101516.5(b)(1) Teacher-Child Ratio was issued today on the attached LIC 809D. Continue to page 2. Page 2 LPAs Navar and Deschampe informed Program Lead Christina Orellana that this report dated 12/18/204 for Type A citation which shall be posted for 30 consecutive days as there is immediate risk to the health, safety, or personal rights of children in care. Also, LPAs Navar and Deschampe informed the Program Lead to provide a copy of this licensing report dated 12/18/2024 Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. Exit interview conducted and report was reviewed with the Program Lead Christina Orellana. A notice of site visit was given and must remain posted for 30 days. End of report.
