Inspector’s narrative

What the inspector wrote

care children (2) Staff handles day care children roughly, and (3) Staff yells at day care children. Reporting Party (RP) stated Staff 1 (S1) hits children a lot, usually by swatting their hands. Staff 2 (S2) was seen dragging a child by the arm, and yelled at children. During the course of the investigation, LPA interviewed 9 staff members and contacted 10 parents/authorized representatives but only 2 responded. Children interviews were attempted but did not qualify with the Ten Step Investigative Interview. Regarding allegation: (1) Staff hits day care children. During the staff interviews, 8 out of 9 staff stated they did not witness a staff hit a day care child. 1 out of 9 staff did not answer the question. S1 stated if a child hits somebody else, S1 normally reacts and maybe slaps the child’s hand. When a teacher brought the kid to the office, S1 sort of hits their hand for the purpose of education. Staff 10 (S10) stated S1 hits the children’s head with S1’s palm while children were practicing for the holiday performance. Regarding allegation: (2) Staff handles day care children roughly. During the staff interviews, 8 out of 9 staff stated they have not witnessed any staff at the child care center handling children roughly. 1 out of 9 staff did not answer the question. S10 stated S2 was seen to yank the arm of a child to move the child away from the other children. During LPA observations on 02/04/2026, LPA observed multiple staff hold children’s wrist or hand when pulling children to a desired location and pulling up children by hand or wrist while they were laying down on the floor. LPA observed staff lift children from under the armpit, as well. Regarding allegation: (3) Staff yells at day care children. During the staff interviews, 8 out of 9 staff stated they have not witnessed any staff yelling at children. 1 out of 9 staff did not answer the question. In addition, 2 out of 9 staff stated they have heard a stern voice but not yelling. S10 stated multiple staff yell at children. During LPA observations on 02/04/2026, LPA observed multiple staff raising their voices. LPA was approximately 22 feet away and heard the staff voices. LPA was in the room next to the office and on multiple occasions heard staff raising their voice/yelling at children. LPA contacted 10 parents/authorized representatives but only 2 responded. 2 out of 2 interviewed parents/authorized representatives did not make any disclosure regarding the above allegations. Based on interviews conducted by LPA, observations, and documents obtained, the preponderance of evidence standard has been met, therefore the above allegations are found to be SUBSTANTIATED. California Code of Regulations, Chapter 1 Title 22, Division 12 Section 101223(a)(1) Personal Rights is being cited a Type A on the attached LIC 9099D forms. Please refer to LIC 9099D for documentation of deficiencies. Licensee was informed of the following: 1. Provide a copy of this report to the parents/authorized representatives currently enrolled by the next business day or immediately upon return. 2. A copy of this report shall also be provided to the parents/authorized representatives of any newly enrolled children for the next 12 months (1 year). 3. Obtain signature and date from the child's parents/authorized representatives on the Acknowledgement of Receipt of Licensing Reports (LIC9224). 4. Keep a record immediately upon receipt of the completed and signed LIC9224 acknowledging receipt of this report in the child's file. The exit interview was conducted with the licensee, An Sook Suh and facility representative,Yooni Kim . The Notice of Site Visit was posted during the visit. The licensee was reminded that the Notice of Site Visit must be posted for 30 consecutive days. Appeal Rights were discussed and provided to the licensee and their signature on this form acknowledges receipt of these rights. Licensee must file an appeal, in writing, to the Regional Manager within 15 business days from the date of receiving the penalty assessment or notice of deficiency. First level appeals should be sent to the Regional Manager at the address listed above. End of report.
