Licensing Program Analyst (LPA) Carmen Odom arrived at the facility for the purpose of a proof of correction visit. LPA Odom met with Administrator Dharmasiri, LPA took census in each classroom, the blue room had 17 preschool age children with 2 staff and green classroom had 16 preschool age children with 2 staff. Facility was observed to be within ratio and capacity. A review of the Facility Personnel Report Summary on this date indicates all facility residents, staff, or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. LPA Odom conducted an unannounced visit on 03/21/24 when LPA cited for Teacher-Child ratio, LPA observed 2 preschool staff with 30 preschool children. LPA cited for Supervision; LPA observed 3 preschool age children were coming out of the classroom restroom alone without any staff supervision. LPA cited for Commingling, LPA observed school age children commingling with preschool age children during lunch, rest time, and outdoor play. During today\342\200\231s visit the preschool staff were in ratio and supervising the children inside the classroom while they were doing activities. There were no school age children present, school age children arrive in the afternoon. Administrator stated they pick up and supervise the school age children in the school age classroom. At 2:45pm another staff arrives to supervise the school age children. Administrator stated they have provided a copy of the report to the preschool families and the LIC9224 Acknowledgement receipts were signed. No citations were issued during today's inspection. A notice of site visit was issued and Administrator Dharmasiri Weerakoon was informed failure to post for 30 days will result in a $100.00 civil penalty fee. An exit interview was conducted with Administrator Dharmasiri Weerakoon and appeal rights were issued and explained.
