What the inspector wrote

On 04/02/26 at 10:30am a case management inspection was conducted by Licensing Program Analyst (LPA) Chan who met with Director Irene Porras. LPA and Director toured the facility inside and outside. The purpose of the inspection is to amend a report dated 02/05/26. Upon arrival there were 13 infants and 5 staff. The LIC809-C report dated 02/05/26 was amended. During inspection, LPA observed changing tables in the 2 infant rooms were not within arm’s reach of a sink. Director stated they have not fixed the issue about the changing table. Headstart representatives Traci Sallade and Vallerie Padilla stated they submitted a second level appeal. The facility is in violation of the California Code of Regulations Title 22, Division 12, Section 101439(h)(4) Infant Care Center Fixtures, Furniture, Equipment and Supplies. See LIC809-D for 1 Type B violation cited. Exit interview was conducted and report and deficiency were reviewed and discussed with Director Irene Porras. The Notice of Site Visit was posted and must be posted for 30 consecutive days. Appeal Rights were provided. Director refused to sign this report. End of Report
