What the inspector wrote

On 02/19/2026, Licensing Program Analyst (LPA) Olivia Meza conducted an initial case management inspection to follow up on a self reported unusual incident that was submitted to CCL Regional Licensing Office on 02/13/2026. A tour of the facility was conducted, and census was taken. LPA observed 121 preschool children in individual classrooms with 13 staff present. According to Fatima Asghar, The unusual incident occurred on 2/13/26 that stated C1 had an unknown injury occur during nap time. During today's inspection, LPA Meza conducted staff interviews and collected documentation. LPA reviewed a copy of the incident report given to the authorized representative. The Daily tracking of nap time which provided documentation that C1 was asleep at approximately 1:00pm. The injury was noticed at approximately 2:45 when C1 was getting up. Three (3) out of three (3) staff interviewed stated that C1 was supervised during nap time and never left alone. S2 stated that they were able to visibly observe and hear C1 during nap time. S2 stated that C1 was not visibly injured prior to nap time. Authorized representatives were informed and interviewed for further information. LPA reviewed the parent handbook which states that "all employees are expected to assist in the prevention and control of injuries, illness, and hazards to ensure compliance with all applicable laws and regulations." The most recent staff meeting minutes conducted on 2/18/2026 stated that staff will be conducting face to name checks during nap time every 30 minutes to ensure the health and safety of children in care. No deficiencies are being cited. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the Director, Fatima Asghar.
