What the inspector wrote

An unannounced case management inspection was conducted on this date by Licensing Program Analyst (LPAs), Nunez in response to a self-reported incident dated 5/15/25. The overall census observed were 2 infant age children and 5 toddler’s that are from the toddler component classroom and 3 staff members. 4 out of the 5 toddler age children that are in the component classroom were placed in classroom 1 in the infant classroom. 1 out of the 5 toddler children that are in the toddler component classroom were placed in the 2 year classroom due to staff members calling out sick. A review of adult records indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. On 5/15/2025 an Unusual Incident Report was filed with the Department to self-report an incident that occurred date is 5/15/2025. The director, Elaina Estrada reported that on 5/15/2025 around 9:15am Child#1 C1 developed a rash on C1’s body. The director called C1’s Parent#1 (P1) to notified about the incident and P1 request the director to call 911. The paramedics arrived at the facility by 10:17am and transported C1 to the hospital along with P1. During today’s visit LPA interview 4 staff members, reviewed C1’s file, and obtained copies of C1’s Illness record, facility’s sick policy, and daily report for C1. Due to insufficient information available at this time, the reported incident needs further investigation. Exit interview was conducted with Assistant Director. The Notice of Site Visit was posted. The assistant director was informed that the Notice of Site Visit must be posted for 30 consecutive days. Failure to post will result in civil penalties of $100. End of Report
