All staff stated if children are still asleep after opening the blinds and turning on the lights, staff will wake the child up by placing their hands on the child back or shoulder and tell the child nap time is over.

    Based on observation and record review, the licensee did not comply with the section cited above. There shall be a clear written statement of the administrative responsibility and authority delegated to the child care center director.

    Based on observation and interview, the licensee did not comply with the section cited above. Children immunization records were unavailable at the time of inspection. Assistant Director stated he did not have access to the files so LPA could review children records which poses/posed a potential health, safety or personal rights risk to persons in care.

    Based on observation, interview and record review, the licensee did not comply with the section cited above. LPA was informed Director (S1) no longer works at the facility. Facility did not have a clear written statement of the administrative responsibility and authority delegated to the child care center director which poses/posed a potential health, safety or personal rights risk to persons in care.
