What Happened
The infant/toddler center was operating within capacity and ratios, with indoor and outdoor areas generally in good condition. Deficiencies were cited for expired mandated reporter training and missing infant sleep plans for two infants.
Observations
- Facility was within licensed capacity and staffing ratios.
- Five staff had current Pediatric CPR/First Aid.
- Five of seven staff immunization records were compliant.
- Five mandated reporter training certificates were expired.
- Two infants did not have completed Individual Infant Sleeping Plans (LIC 9227).

On or before March 30, 2018, a person who, on January 1, 2018, is a licensed child day care provider, administrator, or employee of a licensed child day care facility shall complete the mandated reporter training provided pursuant to paragraphs(2) and (30 of subdivision (a), and shall complete renewal mandated reporter training every two years following the date on which he or she completed the initial mandated reporter training. This requirement was not met as evidenced by LPA reviewing seven staffs' files today at 9:40 am finding 5 staff had expired certificates. The facility failed to meet this requirement of Title 22 Regulations. This is a potential risk to the health and safety of children in care.
