What Happened
Annual inspection found non-functioning sink at infant changing table (out of service for approximately 2 months) and one child missing immunization records. Otherwise facility operating within capacity and ratios.
Observations
- No running water at infant changing table sink for approximately 2 months
- Staff washing hands at different classroom sink instead of at changing table
- 1 child missing immunization records on file

Based on observation and interview, the licensee did not comply with the section cited above. During physical plant inspection, LPA observed a sink within arms reach of the changing table did not have runnning water. This poses/posed a potential health, safety or personal rights risk to persons in care.
